VNA of Care New England recently launched a pilot program with several physicians and their office staff to test a new homecare portal. The secure, online portal enables physicians to access patient data that is extracted from the homecare medical record. Information such as reconciled medication lists, trends in vital signs and progress on treatment plans allows for real-time changes in care to occur, maximizing the potential for exceptional clinical outcomes. The portal also allows for physicians to receive and return required documentation electronically, providing added convenience and time-savings.
Cindy Bielecki, RN, MS, CS, Director of Community Outreach and Provider Relations is the project's coordinator. Currently, she is providing education to the the pilot physician practices and their office staff. Bielecki said, "We are very excited to offer this service to our physician partners. The availability of secure online portals is relatively new in the homecare industry. Providing this value-added technology serves to further distinguish VNA of Care New England from other homecare providers while supporting the collaboration and communication that is essential to better manage patient care."
