Blackstone Health, Inc. is a non-profit organization managed by the VNA of Care New England which facilitates two major programs in the State of Rhode Island: The New Horizons Adult Day Center and the Blackstone Health Café. The New Horizons Adult Day Center provides services to adults who suffer from some form of physical or cognitive disability, while the Blackstone Health Café is comprised of 17 nutrition sites throughout the state wherein meals and nutrition education are provided to individuals 60 years of age or older.

The New Horizons Adult Day Center centers provide structured programs during the day in a community group setting to support the personal independence of older adults and those that are disabled or have cognitive decline. Programs are designed to promote the physical, social, and emotional well-being of participants while providing health care and food services.
This program is the largest congregate dining setting for seniors or for those with disabilities in Rhode Island. The Blackstone Café provides balanced and nutritional meals to disabled adults and seniors in a community dining setting. There are 17 Blackstone Health Café dining locations across the state of Rhode Island where meals can be enjoyed or picked up.
