At the VNA of CNE, our staff is dedicated to providing the highest level of care. The rehabilitation team coordinates the patient's care to ensure all needs are met. Each of the team members will provide the patient with an individualized treatment plan that involves exercises at home to help the treatment progress and maintain gains after discharge from services.
The staff will come to the patients in their home to conduct their treatment. In this environment, we are able to identify specific issues limiting function and create a treatment that takes into account the home's structure to maximize the patient's ability to age in place.
At the first visit the therapist will discuss your medical history which will include a review of your medications. During the visit the therapist will take the patient through an assessment of strength, flexibility, balance and ability to complete functional tasks. During the assessment, the patient’s needs are identified, this allows for the formation of an individualized plan to maximize the patient’s rehabilitation potential.

Medical Social Workers perform an assessment of the patient’s support needs to help maintain the highest level of independence. The Social Worker will educate patients to available services (long term placement, private caregivers, community services, etc). The Social Worker will create an individualized plan for the patient, provide necessary forms and discuss the necessary steps the patient and family need to take to obtain the services.
